Phyllis Tuckwell have launched a Transitions programme, which supports carers when their loved one moves from one place of care to another, for example from hospital to a care home, or from home to our In-Patient Unit.

Carers are usually a patient’s relative or close friend, and adjusting to their loved one being moved to a different care setting can be stressful. This programme offers them emotional and practical support through the process, helping reducing stress and worry. Through phone calls, visits and assessments, we can identify their individual concerns and needs, and provide or signpost them to the support available, helping them feel prepared, included and supported throughout their loved one’s transition.
